How to add a Rating question in Microsoft Forms
For those who are new to exploring Microsoft Forms, you may have noticed the Rating question type. The Rating question type allows users to provide a rating based on how much they like or dislike the scenario you have proposed. The Rating question type is particularly useful in scenarios where participants need to express their opinions or preferences on a scale.
By default the Rating question uses the very familiar star rating system e.g. 1 star or 5 stars however you can easily adjust this to better suit your questions scenario by changing the Symbol used and also how many levels you want to offer e.g. 5 starts or 10 stars.
Add a rating based question
If you are new to using Microsoft Forms, check out how to add text-based questions and multiple choice based questions.
- Login to Microsoft Forms via https://forms.microsoft.com.
- Open an existing form to add the rating question, or click New Form or New Quiz.
- From the form, click the Rating button from the toolbar:
- Add the text to create the question.
- Use the Levels drop-down menu and choose how many options you want to provide for the rating. There is a minimum of 2 and a maximum of 10.
Add an image to a Rating question
Just as you can incorporate images into text-based and multiple-choice based questions, you can also use them with the Rating question type. Images can provide a visual reference to the question or it may make up the main part of the question E.g. How do you rate this photo of XYZ?
- Click the Media icon to the right of the question area:
- The Insert media pane will open on the right side of the screen:
- Choose either Insert Image or Insert Video, for this I will Insert Image.
- You will now see options to perform an Image Search, OneDrive or Upload an image from your computer.
- Select from the options to insert an image.
- The image will now be displayed within the Rating question.
Mark the question as required
If you want to force readers to answer a particular question, you can do this using the Required setting. This can be used within any of the question types as needed.
- From the text question, click the Required button to enable this setting.
Add a subtitle to the Rating question
If you wish to add a subtitle underneath the main question, or even some additional instructions for this specific question, you can activate the subtitle feature.
- From the Rating question, click the ellipsis button (…) in the bottom right corner.
- Select Subtitle from the options.
- The subtitle area will be displayed within the question.
- Add text as needed.
Add Rating Labels
You may wish to add labels to the Rating question to indicate to participants which end of the rating scale is the positive and which is the negative.
- From the Rating question, click the ellipsis button (…) in the bottom right corner.
- Select Label from the options.
- The Label area will be displayed within the question.
- Click the section to Enter a label for each end of the rating spectrum.
Delete a question
If you no longer need to keep a question, you can easily delete them.
- From the rating question, click the Delete button.
- The question will be deleted from the Form.
Preview the Form
To preview the quiz or form, and check what it will look like, click the Preview button on the main menu area.
Rating question examples
- On a scale of 1 to 5, how satisfied are you with our latest product?
- Rate the overall quality of the product from 1 (poor) to 5 (excellent).
- Please rate your experience with our customer support team on a scale of 1 to 10.
- How helpful was the support team in resolving your issue? (1 being not helpful, 5 being very helpful).
- Rate your overall satisfaction with the event on a scale of 1 to 5.
- How likely are you to recommend this event to a friend or colleague? (1 – not likely, 10 – very likely).
- Please rate the effectiveness of the training session on a scale of 1 to 5.
- To what extent did the training meet your expectations? (1 – not at all, 5 – exceeded expectations).
- On a scale of 1 to 10, how would you rate this course in terms of content and relevance?
- Rate the clarity of the course materials on a scale from 1 to 5.
- How satisfied are you with your current workload? (1 – very dissatisfied, 5 – very satisfied).
- Rate your level of agreement with the statement: ‘I feel supported by my team,’ on a scale of 1 to 7.
- Rate the ease of navigation on our website from 1 to 5.
- How would you rate the overall user experience of our website? (1 – poor, 10 – excellent).
- On a scale of 1 to 5, how would you rate the taste of the dishes you ordered?
- Rate the speed of service on a scale from 1 (very slow) to 5 (very fast).
- Please rate the relevance of the presentation content on a scale of 1 to 5.
- How engaging was the speaker? (1 – not engaging, 10 – very engaging).
- On a scale of 1 to 7, how comfortable was your stay at our hotel?
